What is a second stage protein substitute?

A second stage protein substitute is a formula which is more concentrated in Phe-free protein than that of Phe-free infant formula. These protein substitutes enable babies with PKU to meet their protein needs in a much lower volume. Therefore,  they still have an appetite for a wide variety of solid food.

Second stage protein substitutes are available as powder, requiring the addition of water to form either a low volume drink, or a spoonable smooth semi-solid. They are also available ready to drink.

Why does my child need to move on to a second stage protein substitute?

Babies with PKU require a second stage protein substitute as it enables them to meet their increasing protein and nutrient needs.   It also assists in meeting their developmental needs, during the transition from breast/bottle feeding to eating solid foods, and drinking from a cup.  

The transition to a second stage protein substitute is a gradual process and your dietitian will advise.

  1. As the amount of Phe-free protein from second stage protein substitute increases, the equivalent amount of Phe-free protein from Phe-free infant formula decreases. 
  2. The second stage protein substitute is generally given before solids and any Phe-free infant formula still being taken.  This is to ensure it is taken whilst your baby’s appetite and interest are at their highest. 
  3. Water or other permitted drinks should be provided after your child takes their second stage protein substitute.
